If you are interested in sending a gift kit, please download this order form. Forms should be mailed or faxed to us, per instructions on the bottom of the form. A gift card will be enclosed in the Chemo Comfort kit. Please let us know any additional names you would like to have on the card to let the recipient know who the gift kit is from.
